Internal vacancy – applications for this post will only be accepted from current employees of Aberdeenshire Council (this includes relief staff and agency staff who are currently undertaking work with us) and  current employees of NHS.

This new post will support a core team within the Aberdeenshire HSCP to develop and implement a new ‘Hospital at Home’ service for Frail Older people being discharged from acute Hospital care throughout Aberdeenshire. Candidates will ideally have knowledge of care pathways in both Hospital and community settings.

Whilst the post is based at Woodhill house working at home will be expected.

There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through the use of Continuous Improvement and QI methodologies for the benefit of the Service and the organisation.

This post has a minimum requirement of one reference which must be your current or most recent employer.

Fixed Term for a period not exceeding 31/03/2022.
