This post offers an exciting opportunity to work as a Specialist Community Physiotherapist within an innovative integrated Older Peoples Team in West Dunbartonshire HSCP. You will act independently to assess, diagnose, plan, treat and evaluate specialised programmes of patient care for people aged 65 years and over who have frailty, multi morbidities and highly complex presentations. You will maximise rehabilitation potential and support them to remain living at home as safely and independently as possible. The role involves you working as member of a multi - disciplinary team providing a planned and co-ordinated approach to health and social care.
It is essential you have a Diploma / Degree in physiotherapy with current HCPC registration. You should have substantial post graduate experience and relevant experience working within Rehabilitation, Community and Medicine for Elderly. The working pattern is Monday to Friday 8.30am – 4.30pm. This post requires membership of Disclosure Scotland’s PVG Scheme.
