Clinical Fellows within the REACT TEAM
Medicine of the Elderly
St John’s Hospital, Livingston
We are delighted to offer 2 Clinical Fellow posts (Fixed Term 40 hours per week) with the REACT team in West Lothian. Based within the REACT Hub at St. John’s Hospital, the post holders will gain experience in a range of clinical areas including Hospital at Home, Rapid Access Frailty clinic and West Lothian’s community hospitals (St. Michael’s and Tippetthill). In addition, the post holders will contribute to front door frailty assessment, supporting discharge home from acute services, Nursing Home liaison and development of community frailty initiatives.
The REACT Hub offers a Geriatrician led integrated service with a single point of contact for frailty elderly patients during an episode of acute deterioration. Our team consists of geriatricians, nurse practitioners, clinical fellows, physiotherapists, occupational therapists, pharmacists, administrative staff and a social care co-ordinator.
These posts do not have educational approval from the Postgraduate Dean and will not be recognised for training however these posts will provide the successful applicant with a strong foundation in Geriatric Medicine; they will develop the skills to assess and manage acute illness in the context of multi-morbidity, manage complex end stage disease including palliative care, and work closely within the multidisciplinary team under close Consultant supervision..
With a strong emphasis on education, training and quality improvement, there will be a dedicated weekly session in these areas which can be tailored to suit the applicant. Within St. John’s there are SIM facilities which can be used to deliver educational opportunities to medical students and allied health professionals. The post will be weekdays 9-5pm, however there will also be opportunity for additional out of hours experience within medicine at St. John’s. As the post is based within the community, it is essential that applicants hold a valid driving licence. The post will provide excellent experience for those wishing to develop the skills of complex patient management that underpin Geriatric Medicine.
