Working as part of North West Rehabilitation Service, this post offers an excellent opportunity to work within a multidisciplinary team. You will be part of a team providing assessment, treatment, intervention and advice to an older people and adult population with multiple pathologies, supporting hospital admission avoidance , hospital discharges and supporting programs of rehabilitation and patient self-management

The post holder will work autonomously in a diversity of health and social care setting taking on in addition to the clinical role, case co-ordination, health promotion, audit activity and providing expert clinical intervention to patients and education and support to their carers

In addition to this, the post holder will hold a complex caseload applying knowledge and skills appropriate to their professional role within both a multi, and inter-disciplinary team

You will be a Physiotherapist with HCPC registration. A full driving license is essential. Experience of being part of a multi-disciplinary team and working in the community is desirable.

The shift pattern for this post is weekdays and occasional weekends.
