Macmillan ICJ Wellbeing Practitioner - (1.5 x posts) Fixed Term for 3 years
East Dunbartonshire HSCP is working in partnership with Macmillan Cancer Support to launch a new service called ‘Improving the Cancer Journey’ (ICJ). We have an exciting opportunity for two highly motivated and enthusiastic individuals to join our team.
We recognise that a cancer diagnosis can come with a wide and worrying range of potential impacts beyond the purely physical. These challenges include financial, housing, mobility, physcological/emotional, spiritual, relationships and getting access to the full range of NHS services available. Trying to navigate through the many public and voluntary sector organisations who can help those affected by cancer can be difficult at any time however, it is especially so when dealing with the shock and impact of a cancer diagnosis. The aim of the ICJ Service is therefore to provide immediate access to expert information, advice and support to people affected by cancer.
You will work in a ‘Macmillan Professional’ branded role and your duties will include meeting with East Dunbartonshire residents who have received a cancer diagnosis in order to carry out a Macmillan Holistic Needs Assessment. Together, you will develop an Individual Care Plan tailored to their specific needs. These meetings will take place across a variety of locations and settings. You will offer advice, build capacity and support the development of the ICJ service by working in partnership and signposting and referring to appropriate partner agencies and services.
Ideally, you will be educated to degree level but significant experience in a similar role will be seriously considered. You will have strong interpersonal skills and a belief in the strengths and resilience inherent in people and communities. You will, along with the ICJ Project Team will deliver a person-centered service as a core component of the Improving Cancer Outcomes Partnership on behalf of the East Dunbartonshire HSCP.
It is essential that you have experience of: supporting people who have experienced the impact of a long-term health condition, in either a personal or professional capacity; dealing with the public and engaging with public sector, voluntary or other statutory agencies; have a friendly, positive manner with effective people and advocacy skills in relation to communication, listening, supporting and motivating.
In addition, you must have the ability to: manage difficult interactions in a respectful, supportive and non-judgmental manner; organise and prioritise work, meet key deadlines, assimilate complex information, produce written reports and maintain client records through input to IT systems; and, effectively assess the needs of individuals who may be in crisis and engage effectively, sensitively and flexibly.
It is desirable that you have experience in a health or social care-based setting and have a driving license.
The role is offered as a 3 year fixed term appointment and offers a unique opportunity to work with a wide range of partners across health and social care, local authority, third sector and service users and their carers.
