As part of a multidisciplinary team you will have responsibility for delivering Comprehensive Geriatric Assessment (CGA) to older adults on admission to Queen Elizabeth University Hospital and Gartnavel General Hospital. The role will facilitate CGA within ground floor of QEUH and provide medical Liaison throughout the hospital. This role will support safe onward transfer and supported discharge of patients through appropriate screening and patient/family/carer involvement. You will act as an expert resource providing advice where required to members of wider multidisciplinary team. This role will incorporate audit to measure impact of service and identify ongoing areas for improvement. You should be a registered adult nurse with current NMC registration and have the relevant experience in the specialty, Evidence of further education, including post graduate certification/ diploma/continuous professional development in relevant area of practise. You should have good communications skills, ablity to work using own intitative and have good interpersonal skills. There are 2 posts available ( 30.00) and (25.00).
