An exciting opportunity has arisen for a Band 5 Staff Nurse position within Older People Service, North Sector This services has a progressive team working towards a safe and patient assured care during the patient’s journey. Nurses are involved from the assessment at triage stage initially in A&E to ensure the patient’s needs are met. Assessing fragility and planning care within a timely fashion is the Older People Services ethos. The Rehabilitation begins at admission, with the nurses at the hub of organising care and setting standards. You will work within the nursing and multi-professional team to ensure a high standard of care to a defined patient group. This team include apprenticeship candidates, students and rotational nursing teams.  You will provide clinical leadership for the assessment of patient needs, implementation and evaluation of care, which are evidence based. Utilising all available resources taking into consideration the lifestyle, gender and cultural background and ensure involvement with the patient, family, carers and significant others. You will empower patients to take responsibility for their health, well-being and future lifestyle by practicing in an open, transparent and inclusive manner; thereby ensuring patients have the relevant information to participate in decisions about their care. As part of this team your continuing development is forefront with access to further education and leadership development with internal support with vocational modules via SVQ scheme and yearly grants available for conference contributions or Master programmes. It is essential that you are currently a Registered Adult Nurse with  current NMC with post registration experience.  The shift pattern is; 24 hours rotational.
