Community Link Practitioners (CLPs) will provide a person centred service that is responsive to the needs and interests of GP Practice patients within South Ayrshire. You will support them to identify issues that affect patient’s ability to live well.
Working collaboratively with the patients you will support them through decision making processes to help them decide upon the issues that they would like to address and help them overcome any barriers to address these.
Using various action learning tools and creative improvement methodologies the CLP will support the existing General Practice surgeries to adopt a links approach. You will support existing staff in becoming skilled in identifying local services and matching them to the needs of individual patients. Typical examples include: social or lunch clubs, self-help groups, befriending organisations, hobby clubs (such as gardening clubs) and employment or voluntary work agencies.
You will identify community resources and facilitate relationships between these resources and the Practice for the benefit of the patient. You will also build relationships and processes with statutory organisations, health service and voluntary organisations, NHS and charities. You will be educated to HND level, demonstrate highly effective interpersonal and communication skills, ability to form credible links with primary care and other health and social care partnership and third sector providers. You will have a good knowledge and experience of working with people who are experiencing complex social and emotional circumstances. You will have excellent influencing, negotiation and motivational skills in order to engage patients and enable them to take up a range of wider community services and activities.
CLPs will be based within GP Practices and other available community settings within South Ayrshire, often linking with 2 or more Practices in the same geographical community or sharing the same premises.
A car driver is essential
Applicant will be expected to work flexibly within hours stated.
Post 1 - Fixed Term for 12 months
