As an Advanced Nurse Practitioner within the intermediate Care & Reablement team you will be based within Cathcart Street GP Practice Ayr. You will be supporting two Care Homes aligned to the practice, thus linking different sectors of health and social care to better coordinate clinical care for care home residents. You will participate in the provision of high quality clinical care and management services within NHS Ayrshire & Arran.
You will be responsible for delivering expert clinical care and management of both the stable and acutely unwell older person, making complex, autonomous decisions using advanced knowledge and skills, having overall responsibility for the assessment of care needs and the delivery of advanced nursing care to personal caseload.
Working collaboratively with the Care Home care and clinical staff, the multidisciplinary team within the aligned GP Practice(s) for the Care Home, as part of the Health and Social Care partnership, to promote early discharge to Care Homes from acute/community hospital settings; to respond to existing or potential ill health issues in the person’s Care Home, and to contribute to a reduction in unplanned acute hospital admissions. You will need to possess a sound clinical knowledge, good IT skills excellent communication skills, interpersonal skills, and problem solving skills, the ability to work autonomously and effectively as part of the multidisciplinary team.
Qualified non-medical prescriber with advanced assessment skills is essential.
We support development of our staff and applicants not currently holding a practice prescribing certificate will be supported to complete this. NHS Ayrshire & Arran has a large number of pharmacist prescribers working in secondary and to support patient-centred care in accordance with Prescription for Excellence.
You will participate in weekend, public holiday and extended hours rotas as required. In accordance with AFC terms and conditions you may be required to work over 7 days and or an extended working day.
Hours: 37.5 (job share or part time may be considered).
A car driver is essential.
