This post requires the post holder to have a PVG Scheme membership/ record. To undertake the role of a Community Nurse Practitioner, utilising high levels of decision making and clinical judgment to ensure that patient health needs are met through the provision of the management of patient conditions through assessment, treatment, evaluation and review planning ensuring the delivery of high quality person centered care to patients in partnership with patients, families and carers.

Provide specialist advice to healthcare professionals, other agencies, carers, clients and relatives. The Community Nurse Practitioner role encompasses that of specialist nurse and educator.

The Community Nurse Practitioner provides day to day leadership for the delivery of patient care for a defined caseload and allocated nursing team including supervision and deployment of staff.

Support the development of the service to improve practice and work collaboratively across healthcare boundaries, implementing and evaluating evidence based standards, guidelines and policies.
