This is an exciting opportunity for an enthusiastic and self-motivated pharmacist to join the Prescribing Team within a Health and Social Care Partnership (HSCP) setting and take forward key initiatives. As a member of the Prescribing Team you will have an important role in providing individualised pharmaceutical care to older patients and patients living with frailty, improving the transfer of medication related information across the primary/secondary care interface, promoting safe, high quality evidence based and cost effective prescribing and supporting a range of Health and Social Care Teams to optimize safe and effective use of medicines. This patient facing role will involve taking on a prescribing role and developing your own clinical caseload in line with Achieving Excellence strategy objectives. Applicants should be qualified independent prescribers or be prepared to work towards this qualification. Your role will be to work as a member of the NHS GGC prescribing support team. The objectives of the team are to improve the quality and cost effectiveness of prescribing and medicines use for patients across NHS Greater Glasgow and Clyde. You will work closely with GPs, other practice staff, community pharmacists, care home staff, members of the primary healthcare team and central team colleagues in supporting the development and application of high quality, safe, cost effective, rational and evidence based prescribing in the HSCP. You should have a sound clinical knowledge, good IT skills, excellent communication and interpersonal skills and be able to work autonomously as well as part of a team. You will work on joint projects/initiatives within the HSCP as required and promote awareness of prescribing management issues. You must be registered with the General Pharmaceutical Council and be able to demonstrate a commitment to Continuing Professional Development. You must have significant post registration experience. You will be required to travel between GP practices and care homes to carry out the duties of this post. Experience of directly working with primary care teams and knowledge of GP systems would be advantageous. This post requires PVG Scheme Membership/Disclosure Scotland Check.
Minimum requirements
Masters Degree in Pharmacy or equivalent
Registrant with the General Pharmaceutical Council
Independent prescriber or working towards
Minimum of 2 years’ post-registration experience
The shift pattern is Monday to Friday 8.30am to 4.30pm
