Due to demand, this new team is being expanded, providing an exciting opportunity to be involved in promoting independence and reviewing packages of care and support in the community.
Physiotherapist Band 6
This permanent post is dedicated to improvement: to review and reduce the number of packages of care in the community, to work with community care staff and contribute positively to support earlier discharge for individuals from hospital, and to enable their independence in the home environment.
This will involve assessing an individual’s functional abilities; working directly with care providers to create a culture of enablement, independence and supported self-management; and reviewing and reducing unnecessary packages of care.
It has been recognised that there are an increasing number of packages of care prescribed for individuals who have rehabilitation potential which is not being realised. This post works in partnership with the person, their family/carers and multi-agency teams to achieve the best functional outcomes. Hours of work are: 37.5 hours per week.
Applicants must be HCPC registered. As services may be provided over seven days in order to support patient flow as well as to test different delivery models, flexible working hours may be required as part of the post.
