An opportunity has arisen for a registered nurse to join the health inequalities nursing service based within the health and homeless outreach team. You will play a pivotal role in engaging with those who are homeless, at risk of homelessness and other vulnerable groups delivering targeted assertive outreach anticipatory care assessments.

You will have excellent interpersonal and communication skills and have the knowledge of health inequalities and how this affects individual and community health. The ability to demonstrate high quality person centred care and a commitment to multi agency working is essential along with the ability to adapt and manage change within the service. 

The nursing service is under review therefore the role will evolve for the needs of the client group.  

You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the NMC Register

Hours of work are:  37.5 hours Mon – Friday with a degree of flexibility to address the needs of the service
