An exciting opportunity has arisen for a suitably qualified and experienced individual to lead and manage Community Nursing and Specialist Services in Fife Health and Social Care Partnership.
You will be accountable to the Head of Community Care Services, responsible and accountable for managing, developing and providing leadership to the following community health services and specialist services across Fife, within the Health and Social Care Partnership:
• District Nursing
• Home Based Healthcare
• Intermediate Care Teams
• Diabetic Retinopathy
• Long Term Conditions Management
• Vascular Services - Cardiac Nurse Team & Diabetes Nurse Team
• Managed Clinical Networks (MCN’s)
• High Health Gains Team
• Enhanced Community Team
You will be an inspiring leader with highly developed communication and organisational skills, particularly in influencing and negotiating, motivating and managing change.
Experience at a senior level within either local authority, the NHS, or in the voluntary or private sector is required.
Passionate about quality and person centered care, a good understanding of the local community and resources are necessary as well as the ability to work across health and social care.
The post will provide the opportunity to contribute to the continued improvement and development of integrated community services for Fife's residents.
