Home First – Edinburgh Health and Social Care Partnership


An opportunity has arisen for a dynamic and forward-thinking health and social care professional to join the Home First team within Edinburgh Health and Social Care Partnership (EHSCP). The Home First project sits within the Conversation 2 programme, as part of the Partnership’s ambitious Transformation Plan.  The scope of this project is very broad and complex and is one of the Partnership’s most scrutinised and highly visible workstreams. 

As part of a developing Home First project team, the post holder will lead the development and delivery of improvement programmes to support service transformation through delivery of a range of high impact changes with a specific focus on Unscheduled Care and Home First.

Experience of change management within the health and social care sector will be required to develop local capability to ensure sustainable service improvement longer term.  The postholder will be required to work closely with a wide range of different services across EHSCP and NHS Lothian to facilitate improvements in both patient flow from hospital and prevention of admissions from community.
