Assistant Community Practitioner – Band 4
An exciting opportunity has arisen for 3 Assistant Practitioners to join the integrated Aberdeen City Health and Social Care Partnership.
These brand new roles based within our community nursing teams will complement our innovative and inspiring community nursing model designed to facilitate transformational change in improving the health of our local population and delivering high-quality, integrated and person centered care.
Successful candidates will participate in the planning and carrying out of holistic, protocol-based care under the direction and supervision of healthcare professionals, and will assist and support the multidisciplinary team in the delivery of high quality care. Opportunities to work and learn alongside our other community services will be available.
Successful candidates will develop self and other HCSWs across the four Pillars of Practice for HCSWs (Clinical Skills, Facilitation of Learning, Leadership and Service Improvement), and act as mentors and role models.
Candidates must have experience of working in Health or Social Care and have, or be willing to work towards, a relevant qualification at SCQF level 7-8, e.g. HNC / HND / SVQ / PDA.
