The Care Home Hub offers a new and exciting opportunity to work in partnership with Care Home and HSCP Teams across GGC to drive and develop improvements for both residents and staff. NHSGGC Corporate Nursing, Midwifery and AHP team are looking to recruit motivated, passionate, committed individuals with a particular interest in Quality Improvement.

The post holder will work as part of the multidisciplinary team with responsibility for leading on policy development, implementing, and monitoring current guidance/practice on food, fluid, and nutrition within care homes and care settings.  The post holder will support the delivery of food, fluid, and nutrition education and training for staff and have a clear focus on raising awareness of the importance of good FFN management. 

The post holder will have extensive knowledge of food, fluid and nutrition, and will take a lead role in the development of food fluid and nutrition standards, guidance and improvement within care homes and care settings.  The post holder will have a lead role in overseeing dietetic audit processes and developing nutritional action plans that will support care homes and care settings to meet the nutritional requirements of care home residents.  The post holder will work alongside the care home collaborative team to identify training needs relating to food fluid and nutrition, and support the training of teams and individuals.

This role offers the opportunity to work in partnership with Care Home staff and other HSCP and Primary Care colleagues in the ongoing provision of enhanced services for residents within care homes and care settings.

The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience to enable the Team to provide a quality multidisciplinary service.

The post holder will provide professional advice and direction to other professional colleagues and team members and supervise less experienced staff as well as Support Workers and students on clinical placement.
