An exciting opportunity has arisen for an enthusiastic and motivated individual to join our excellent Discharge Team in University Hospital, Ayr.

You will possess excellent interpersonal, communication and organisational skills as well as being able to work effectively as part of a team.

Without supervision but under direction of the Discharge Co-ordinator and in collaboration with the multi-disciplinary team, you will be accountable for the assessment, development, implementation and evaluation of programmes of work for a defined caseload of patients who meet the Home First criteria. You will work within the professional standards held in the NMC Code of Professional Conduct or HCPC Register. You will also work automatously in the effective and efficient management of all patients who meet the criteria for Home First.

You will play a pivotal role in in supporting multidisciplinary ward staff and Health and Social Care Partnership colleagues to ensure patients who meet Home first criteria are discharged in a safe and timely manner.

You will act as part of the Home First Team and co-ordinate the actions required to complete a successful discharge. Coordinating with Physicians, Physiotherapists, Occupational Therapists and other individuals in the patient’s care plan. You will be required to participate in audit and quality assurance programmes, and contribute to the agreed initiatives and champion the Home First ethos.

You will have considerable, relevant clinical experience within an Acute setting and will demonstrate evidence of continued professional development.

You will be responsible for developing and supporting the Home First Service within University hospital Ayr (UHA) with links to the Health and social Care Partnership and Community Hospitals. The post holder is managed by and works closely with the Discharge Co-ordinator and is expected to carry out site wide tasks as required to promote the Home First.

You will be responsible for establishment and monitoring hospital wide audit in relation to Home First. You will also be responsible for supporting the development of discharge planning strategies and initiatives.
