We have an exciting opportunity to join a new multidisciplinary Home First Team which aims to reduce delayed hospital discharges by assessing needs and providing rehabilitation in the home setting.
The home first pathway has been evidenced nationally to result in more timeous, effective and sustained discharge, and to reduce the need for care packages through maximisation of individual independence. The Home First Model is currently being established across South Lanarkshire to support this process.
We are looking to recruit:
- 1 Band 6 Occupational Therapist
- 1 Band 6 Physiotherapist
- 1 Band 5 Occupational Therapist – post will commence in the Home First Team however this is a rotational post and the post holder will be expected to participate in the NHS Lanarkshire OT Physical Care Group rotation
- 2 Band 4 Generic Assistant Practitioners
These posts will initially be based in a central team at Hairmyres Hospital, however this may change as the team is tested and developed.
The posts will provide an opportunity to contribute to the development of this new service and inform future delivery of the Home First model across South Lanarkshire.
The purpose of this role is to work under the supervision of registered therapy staff to assist patients to achieve their rehabilitation goals; to undertake baseline assessments within clearly defined parameters for the identified service and to implement, adapt and progress treatment programmes within agreed protocols and defined parameters.
The successful candidate will possess HNC in healthcare, education or social cares or SVQ III in healthcare, education or social care or equivalent experience in a health, education or social care environment. Ideally the post holder will evidence short specialist courses relevant to specific clinical area.
The post holder will be a Full driving licence holder.
