Dumfries and Galloway Health and Social Care Partnership is undergoing a period of transformational change that is much needed and long awaited. On the back of this a new opportunity has arisen to lead whilst inspiring others, manage and develop the provision of the Home Team service as its delivery model evolves to meet the needs of Health and social care.
You will provide leadership to an integrated team who have a primary responsibility to provide high quality professional standards of care for individuals to:
• Undertake a comprehensive community assessment which identifies peoples immediate and longer term needs, delivering intermediate care in the person’s own home
• Home first approach, preventing unnecessary hospital admissions and bringing people home from hospital earlier in their journey
You will be an inspiring leader with highly developed communication and organisational skills, particularly in influencing and negotiating, motivating and managing change. You will be instrumental in driving the home team agenda forward in Dumfries and Galloway, ensuring that our vision of “People living happier, healthier lives in Dumfries and Galloway” is realised.
You will contribute to the continued improvement and development of Home Teams for Dumfries and Galloway residents.
Experience at a senior level within either Local Authority, the NHS or in the voluntary or private sector is required.
In return, you will be supported by your senior colleagues to help drive change as together we can make real change happen You will also be eligible for further leadership courses and have the opportunity to share our way forward with other boards as NHS Dumfries take up the challenge to lead new developments in the area of Home teams.
