An exciting opportunity has arisen for 3 Registered Nurses to join the Discharge Hub team as we develop a 7 day service supporting patient flow across the Aberdeenshire Health and Social care partnership.
The post holder will facilitate the safe and effective discharge of Aberdeenshire patients from an inpatient care stay at Aberdeen Royal Infirmary (ARI) to home, community rehabilitation & enablement, community hospital and other care setting in a timely efficient manner, whilst ensuring minimal length of stay in an acute hospital setting. The post holder will also support the roving discharge team which is being rolled out across the acute sector to help embed realistic medicine and a ‘home first’ approach.
The post holder will work with Multidisciplinary Teams and Care management colleagues across the Aberdeenshire Health and Social care partnership in order to ensure safe, effective and timely discharges.
We are looking for individuals who are able to commit to working over the 7 day period, various hours are available for these posts. 1 x 30 hour post and 2 x 22.5 hour post.
