This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
Lead the operational and professional management of the integrated multidisciplinary health and social care teams within the WLHSCP Integrated Discharge Hub.
Lead the implementation of a Home First approach across West Lothian HSCP, contributing to the prevention of admission and the facilitation of early discharge in line with local and national aims and objectives of the strategic direction of the HSCP.
Manage the service in line with statutory, NHS Lothian and West Lothian Council requirements, ensuring compliance with budgetary control while allocating resources effectively to achieve positive outcomes.
Responsible for standards of practice, provision of professional guidance, advice and direction to IHD staff ensuring effective multidisciplinary decision-making and appropriate escalation channels are in place where matters arise within the service that require immediate oversight or direction from the Chief Social Work Officer, Chief Nurse, Allied Health Professional Lead or other relevant professional body.
