As a part of the multi-disciplinary Intensive Home Treatment Team, the post holder will work closely with all disciplines/agencies. The post holder will provide a service to individuals with complex mental health, physical and social care needs who present in crisis and who require intensive mental health care or treatment at home, as an alternative to hospital admission. The post holder will work closely with in-patient staff to facilitate the early discharge of individuals in hospital and will be expected to assist in discharge planning and immediate follow up from acute mental health in-patient care. The post holder will to carry out ongoing assessments, including risk assessments, of the individual during the care they receive from the IHTT and inform the consultant psychiatrist and their line manager of any concerns they have from these assessments. The post holder will be expected to liaise with external agencies to update them on the individuals care and to refer on to appropriate agencies when appropriate.

The post holder will be required to supervise the practice of junior staff and students, demonstrating exemplary practice in delivering care to individuals.

The post holder will practice within the legal and ethical frameworks according to appropriate codes of professional conduct, mental health legislation, child and adult protection legislation and contractual obligations.

This post is for 37.5 hours a week and the post holder will work 5 days out of 7, Monday to Friday 9am to 8pm, Saturday and Sunday 9am to 5pm on a rotational shift pattern. This post requires PVG Scheme Membership/Disclosure Scotland check. A car driver is desirable to carry out visits in the community, either within community resources or the service user’s home.

As part of NHS Greater Glasgow and Clyde's pre employment checking for all appointed candidates, evidence of current NMC registration will be required prior to start date.
