You will provide support and timely intervention to patients with chronic respiratory disease in the community. You will prevent unnecessary or unscheduled acute hospital admissions and provide specialist respiratory care to patients with COPD. You will provide clinical expertise and support to healthcare colleagues working in respiratory care field. You will support the early discharge of COPD patients from hospital. You will provide anticipatory care services to patients with COPD at most risk of admission, linking with the Respiratory Early Supported Discharge (ESD) team, and hospital based respiratory assessment services/respiratory nurse specialists. You will promote self-management of the patient living with COPD. You will be part of a multidisciplinary team, working within the team to treat each patient holistically. It is essential that you have a Degree or Diploma in Physiotherapy and should currently be registered with the HCPC.. You should have relevant post graduate experience including respiratory experience. There are two posts available, 1X Full time, . The shift pattern is Monday - Friday 8.30am - 4.30pm with the potential for weekend work.
