The Anticoagulant Nurse is responsible for carrying out Nurse-led Anticoagulant clinics in hospital, outreach and domiciliary visits under managerial direction from Team leaders. To develop and maintain effective communication links between Hospital clinicians, GP’s, Dentists and other disciplines. These links include internal and external agencies and working across the primary care/secondary care boundary.
- To conduct domiciliary visits.
- To provide a high standard of patient care in the monitoring of anticoagulant therapy,
dosing, appointment and record-keeping, patient education and counselling.
- To take responsibility of clinic and house visit sessions, and offer advice when required to other agencies eg. Dentists, GPs.
- To educate and counsel patients newly prescribed anticoagulant therapy.
It is essential that you are currently a Registered Nurse with the NMC and that you have a current driving licence. Clinical experience working with patients who require anticoagulation therapy is desirable.
Excellent and well developed interpersonal; communication and organisation skills are paramount due to the scattered nature of the team across GG& and links with Primary Care. The shift pattern for this post will be via the rota.
