This post is part of an Integrated Health and Community Care Team within West Dunbartonshire HSCP providing rapid access to Focussed Interventions and/or Advanced Care Planning. It offers the opportunity to lead and manage a team of multi disciplinary professionals and support staff, who works with all adult care groups. The team provides direct clinical care aimed at hospital admission avoidance. Holistic assessment and management is carried out in patients own homes or homely setting. Close joint working with the existing Hospital Discharge Team is key to the development of this team and the role of the Team Lead, and will also provide peer support to the successful candidate. This team aim to support adults and older people who are frail, or with multiple complex needs, to remain at home during an episode of ill health that could otherwise require admission to hospital or a care home. This team also incorporates the role of the Care Home Liaison and COPD/Respiratory Nurses within it.
Minimum requirements (see person specific specifications)
First level Registered Nurse or Physiotherapist with currently valid registration with the Nursing and Midwifery Council or HCPC, GPhC
Non Medical Prescribing (V300) qualification (or willing to work towards this)
Advanced Assessment and Clinical Decision making module (or willing to work towards this).
Minimum of 5 years’ post-registration NHS experience including 4 years experience in a Band 6 role or above
Full Drivers licence
