Job Purpose

To provide effective and efficient support and administration services to the Site and Capacity Patient/Client  Flow  - team, often being the first point of contact in the identification, tracking and recording of patient information which relates to the effective management of patient/client pathways through the hospital and community care system.

To support the site and capacity team to co-ordinate, allocate and prioritise boarding referrals and those delayed in hospital in line with the delayed discharge targets. To co-ordinate, plan and record ‘moving on’, Delayed Discharge and MDT/Case Conference meetings and prepare action plans with definitive timescales/deadlines.

To capture and collate statistics in relation to patient flow and prepare, present and illustrate statistical information for reports. This will also include the validation and verification of the monthly Delayed Discharge census.

The postholder will provide a full range of administrative support of a consistently high standard to the delayed discharge patient flow  team, including acting as first point of contact for any enquiries and for feedback about the progress of patents/clients through the pathway.

The postholder will be responsible for maintaining all appropriate databases and systems, including training users in their operation. Will be responsible for providing feedback to appropriate practitioners and managers within the Partnership.

This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
