Children’s Hospices Across Scotland (CHAS) is the sole provider of hospice services in Scotland for babies, children and young people with life-limiting conditions.  We currently have an exciting opportunity within our Family Support Team for a Transition Worker at Robin House, Balloch.

 We are looking for a dynamic and enthusiastic Transition Worker to work alongside a highly motivated and skilled Family Support Team. You will carry out assessments in order to support the holistic needs of young people aged between 16 and 21 with palliative care needs. Advocacy is a significant part of this role, to support young people and their  families to have their voices heard and choices respected as they transition from children’s to adult services.  Support will be offered within the family’s home, hospice and community settings.

 You will ideally be qualified to HND level in a relevant subject area, and will have palliative care experience of working with young people with complex care needs and excellent knowledge of legislation relating to transition, informing community care packages, self directed support.  You will have strong communication skills with the ability to manage difficult conversations and sensitive information.  You will hold a caseload and be able to work autonomously as well as being an enthusiastic team player.  You will need to be highly organised with the ability to deal with competing demands  in order to co-ordinate, develop and deliver a service provision designed to assist with a young person’s transition journey, with recognition of the impact  this may have upon their wider family.

 At CHAS, staff and volunteers work together as one team, focused on the common goal of reaching every family in Scotland who needs our support. We have a 3:1 ratio of volunteers to staff members in CHAS. All CHAS staff will at some point work with or alongside volunteers and will be expected to support volunteers.
