An exciting opportunity has arisen for experienced Healthcare Support Worker or experienced Carer to join our Intermediate Care Team in the Integrated Community Assessment and Support Service (ICASS) in West Fife.
The Intermediate Care Service provides rehabilitation and enablement to the adult population mainly within the community in the individuals own home. The primary function of this role will be to work closely alongside NHS Fife and Fife Council staff to deliver therapeutic treatment and care programmes to service users living in their own homes, preventing hospital admission and supporting early discharge from hospital. Occupational Therapists, Physiotherapists, Nurses and Social Care Staff will provide specialist assessments to this patient group and will provide the post holder with workload allocation, supervision and development.
You should have good organisational skills, excellent communication skills and be able to work independently as well as within the team. Good IT skills are essential to support electronic record keeping and scheduling. You are required to have an SVQ Level III in Health and Social Care or similar subject or equivalent skill level with the desire to complete this training on entry to post.
Working shift patterns to include evening and weekend working will be required. There may be occasions where you will be also be required to work within another ICASS base across the West Fife services.
Our service offers regular supervision, PDP and supports training opportunities. The ability to travel throughout the area in a timely manner is essential to support the clinical and rehabilitation needs of our patient group.
If successful in your application and you do not already hold an SVQ Level III, you will be offered the opportunity to undertake this in Health and Social Care, approximately 9 months after taking up post. It is expected that you complete this qualification, which will be fully funded by NHS Fife. Your Line Manager will provide you with more information once you are in post.
