The Midlothian Flow Hub has been set up to improve the patient journey through the Health and Social Care system. The aim of the Hub is to give every Midlothian patient over the age of 65 a positive experience as they proceed from the acute phase of illness, to rehabilitation, to discharge.


A full time Flow Hub Assistant is required to join the Midlothian Flow Hub to support the expansion of the team and improve patient experience and journey within the Health and Social Care System. The succesful postholder will use guidelines and pathways to provide an urgent referral service & single point of contact for Midlothian Intermediate Care Service, and to clinicians within Midlothian to advise on the correct pathway /site.


A requirement of this role is for the successful applicant to be a member of the PVG Scheme for Adults and Children.
