We are seeking a highly motivated individual to provide post discharge rehabilitation and ongoing surveillance of cardiac patients. This post encompasses both cardiac rehabilitation and heart failure management services. Ideally, you will have sound cardiac skills and knowledge. You will be required to see patients in clinics and manage their care autonomously. You should have non-medical prescribing qualification, or a willingness to undertake this
