Job Advert
These posts offer an exciting opportunity for ambitious and committed Consultants in Geriatric Medicine to develop innovative, holistic, multidisciplinary services for older people in Ayrshire with a focus on frailty assessment and early intervention.
These posts will further build on our project involving the Frail and Older People multidisciplinary team (MDT) and redesign of the patient journey. This focuses on early assessment and intervention that is patient centered and based around comprehensive geriatric assessment. This is the Ayrshire and Arran Frailty Pathway. Ongoing growth of this MDT Frailty Team will support these new ways of working and positively challenge existing pathways of care.
We want to transform the delivery of acute care for people with frailty related conditions enabling us to offer safe alternatives to hospital admission and better support them in their usual homely setting. This includes both our Frailty Team at the front door and our emerging Ayrshire Hospital at Home. This aligns with Scottish Governments 2020 vision. Nine acute care of the elderly (ACE) practitioners are established in post across the 2 acute sites working alongside consultant geriatricians and other clinical colleagues.
The posts will be based at University Hospital Crosshouse, with options to work within our Combined Assessment Unit, Hospital at Home, and supporting other front door and inpatient areas within our acute and community hospitals. There is also the opportunity to support patient care within our community hospital at Woodland View, Irvine and develop any specialist interest that the candidate may bring – options available include orthopaedics and trauma alongside an established consultant geriatrician colleague, peri-operative medicine, medical education, and movement disorders.

When hospital treatment is required, the focus will be on ensuring that people get back to their home or community environment as soon as appropriate, with support that minimises risk of re-admission. Whatever the setting, care will be provided to the highest standards of quality and safety, with all decisions being person-centred.
The post holders will also provide ongoing leadership and training in the expansion of innovative training posts that are community facing which offer exciting opportunities for doctors within the GP training programme and Clinical Development Fellows to work at the interface between Acute, Community and Primary Care Services, within a multi-disciplinary team.
