The Practice Development Nurse (Primary Care) will be responsible for developing and enhancing specialist nursing knowledge, skills and competencies within the Primary Care Teams across the three prisons in NHS GGC.  The Post Holder will make appropriate use of the latest evidence to develop the practice of nursing staff within primary care teams and will be expected to supervise practice, audit clinical care, teach and support nursing staff and to provide skilled professional leadership and guidance.
The Post Holder will bring a pro-active influence imparting knowledge in a range of innovative ways of improving practice and ensure primary care nursing staff are confident, competent, skilled and safe practitioners.  The post holder will be responsible for sharing good practice in all aspects of primary care and treatment across the three sites in NHS GG&C prison healthcare.  The post holder will lead and participate in joint inter-agency training on all aspects of primary care and treatment.
The post holder will be responsible for overseeing the development, implementation and evaluation of clinical guidelines, procedures and policies in NHS GGC Prison Healthcare.  The post holder will work with the Professional Nurse Lead and Service Manager to ensure that the training and development needs of nursing staff are met within primary care teams in prison healthcare.
Essential training and experience required to do the job:

Registered Nurse – level 1 NMC registration ( Adult Nurse) with a minimum 2 years’ experience as a Band 6 Nurse
Educated to minimum degree level/or equivalent

Minimum of 5 years’ experience in primary care/adult/acute nursing
Knowledge and skills of clinical governance
Ability to lead, plan, deliver and evaluate training sessions

Have sound analytical and communication skills
Evidence of ability to work outwith immediate sphere of responsibility
Evidence of ability to function effectively under pressure and meet deadlines

Extensive working knowledge and understanding of the Clinical Governance agenda

Demonstration of analytical and critiquing skills
Possession of teaching skills and ability to contribute effectively and appropriately at meetings, forums and specifically to lead where appropriate
Adept interpersonal skills
Possession of a full range of advanced clinical skills in assessing, managing and evaluating care and treatment of patients with complex physical health needs
Specialist knowledge of value-based practice
Evidence of using digital technology, information systems, email and basic applications
Ability to form therapeutic relationships with others and work as a member of the wider team
Working knowledge and demonstrated evidence of the NMC code

Ability to undertake a number of scheduled appointments and site visits in a variety of environments which involves car driving
Desirable: Experience in approaches in working with patients within primary care settings
Awareness of primary care health and social care needs, including long term conditions and palliative care
Actively working towards post-graduate level or equivalent education
