Hours: 37.5 hours per week (job share considered) Location: Pharmacy, Aberdeen City HSCP. |
This post would suit pharmacists who have experience in any sector of pharmacy and wish to progress in a clinical role within primary care and, in particular, GP practices. With support from more senior pharmacists, you will undertake the following roles within GP practices and primary care: medication/polypharmacy reviews; medicines reconciliation; actioning hospital discharge letters, actioning acute and repeat prescription requests; providing prescribing advice to any member of the health care team; reviewing prescribing data; undertaking audit; answering ad-hoc queries; multi-disciplinary team meetings; and supporting social care colleagues with medication-related issues in care at home services.
Dependent on experience, you will undertake either the Pharmacist Vocational Training Programme (Foundation Framework) or the General Practice Clinical Pharmacist Competency & Capability Framework. All pharmacists with be required to qualify and then work as an Independent Prescriber.
All candidates must have a degree in Pharmacy and be on the practising register of the General Pharmaceutical Council (GPhC); have a broad clinical knowledge; have good IT skills; have excellent communication and interpersonal skills; be a car owner and driver; and be able to work in any location in Aberdeen city.
