DISCHARGE/FLOW MANAGER
BAND 7
Salary (£39,693- £46,467)
An exciting opportunity has arisen for an enthusiastic and motivated individual with relevant health or social care experience to work as a discharge/flow manager in the North Area (Sutherland & Caithness).
Based in the community, working directly with the integrated teams to proactively prevent avoidable admissions (no clinical requirement), facilitate patients being discharged from acute services/community hospitals into community and/or social care provision.
Ensuring that discharges from the services for individual patients are managed in an integrated way and provide a fast and efficient pathway for each individual patient. To assess and identify issues within the patient pathways, across all services.
You will have good communication, management and organisations skills to allow you to work with the wider multi-disciplinary team. You will need to have the ability to work alone and on your own initiative, but still be a team player. You must be adaptable, self-motivated and able to respond to the challenges that this role will provide. Relevant experience working within both the hospital and community environment would be advantageous.
