CALLING ALL OCCUPATIONAL THERAPISTS AND PHYSIOTHERAPISTS
The Intermediate Care Team (ICT) is a multi-agency team, working together with patients and carers to provide rapid assessment of service user’s needs. ICT offer an alternative to hospital admission by maintaining people in their homes and to facilitate the smoothest and earliest possible discharge from hospital. The multi-disciplinary team provides assessment, short term intensive rehabilitation and homecare packages. We have opportunities for highly motivated individuals to join our busy, enthusiastic team, committed to delivering quality services working closely with colleagues across professions and agencies.
These posts are vital to support the wide and complex caseload of patients that the multidisciplinary Team visit and assess.

We are looking for a motivated and enthusiastic Individual to join our dynamic team. You will provide a service to patients in their own homes.
Good organisational and time management skills and an ability to be flexible in day-to-day working to ensure prompt and satisfactory outcomes. You should also have the ability to work unsupervised and have excellent communication and interpersonal skills, written and verbal.
The post holder will required to evidence relevant and proven experience in directly working with clients in a healthcare or homely setting.
The post holder should have a relevant degree in Occupational Therapy or Physiotherapy along with community experience.
Experience of working within a multidisciplinary team and knowledge of the health and social care services is advantageous.
A driving licence is essential for the requirements of this role.
Hours: Various on offer, both full time and part time.
