The Community Nurse Learning Disability (CNLD) covers the geographical area of Lochaber (4,700 sq Km). The area comprises small, remote and scattered rural communities, some of which are only accessed by single track roads and ferries. The population profile is approximately 22,000.
The CNLD operates an open referral system for people with a learning disability, their families, carers and significant others. Referrals can be received from fellow health professionals, local authority and voluntary sector agencies as well as family and self-referral.
A five-day-a-week service is provided, which links in closely with the 11 GP practices and primary health care teams across the area to provide a multi-disciplinary approach to patient care. Strong links are forged with New Craigs Hospital, Psychiatric and Psychology services along with Social Work colleagues and the Voluntary agencies. In-Hospital services for people with learning disabilities are sited in Inverness.
The post holder is responsible for managing their own caseload working with clients of all ages (adults and children) with a Learning Disability who are resident within Lochaber providing Health Assessment, Care and Planning. Providing support and advice to other agencies, statutory and non-statutory. The post involves extensive travel within the defined remote and rural geographical area, and includes a significant amount of domiciliary visiting.
