An exciting opportunity has arisen to work within Perth & Kinross Health & Social Care Partnership (HSCP) in a senior clinical nursing role, as part of the redesign of care for adults and older people. We are developing our locality models further with the introduction of a Hospital at Home Model and development of our Urgent Care Pathways.

The Lead ANP role offers an excellent opportunity for an enthusiastic and dynamic Registered Nurse already functioning at Advanced Practice level to improve outcomes and therapeutic experiences for Older Adults by providing strong, expert clinical and leadership across the HSCP.

The post holder will play a lead role in the leadership and development of advanced nursing practice roles under the direction of Nurse Consultant and/or Locality Manger within the Locality Integrated Care teams and supervise and line manage existing ANPs.

You will also develop and participate in the delivery of education and training programmes to support early intervention and anticipatory care approaches across the Partnership, working in collaboration with MFE Consultants, GPs and hospital based frailty services. 

 The postholder will work collaboratively with health and social care colleagues across the Partnership to deliver high quality evidence based and person centred care to older people across Perth & Kinross.

You will be an experienced Registered General Nurse with significant post registration experience and appropriate further education in a relative field, and the personal drive, clinical knowledge and commitment to undertake this challenging and rewarding role. You will possess highly developed communication skills and have experience of working effectively across professional boundaries. You will be expected to work autonomously and demonstrate proven leadership and change management skills.   The ability to travel across Perth and Kinross is essential.  Please see job description for other requirements of this post.

You must be registered on Sub-part 1: RN1 or RNA or Sub-part 2: RN2 of the NMC Register
