An exciting opportunity has arisen to work within the Community Mental Health Teams in Perth & Kinross as part of the redesign of care for adults and/or older people with Mental Health Disorders. This new post will be based within the South Perthshire locality with a remit potentially extending across the P & K sub-locality boundaries.

The MHANP role offers an excellent opportunity for Registered Nurses to improve outcomes and the therapeutic experience for individuals with Mental Health disorders within our community settings. The post holder will initially identify and support people with rapidly deteriorating conditions as well as complex issues. The post will be embedded within our Community Mental Health Teams and respond to rapidly deteriorating patients in order to help prevent hospital admission. The post holder will help ensure a whole system approach.

The post holder will pro-actively manage and review individuals who are suffering acute and complex Mental Health distress and, in situations of deterioration, assess and provide a differential diagnosis and develop a treatment plan, working in close collaboration with the Consultant Psychiatrist, wider CMHT and General Practitioner (GP). They will work closely with people and their families/carers to identify clinical issues quickly and aim to prevent avoidable deterioration and admission/ readmission to secondary care. They will ensure all patients under their remit have Anticipatory Care Plans. 

The post holders will also interface with In-patient services to facilitate timely discharge and smooth, efficient and person centred transitions of care. 

The post holder will play a key role in the leadership and development of advanced nursing practice within the Mental Health services across Perth and Kinross, will participate in the delivery of specialist education and training programmes, and work collaboratively with health and social care colleagues across the Partnership to deliver high quality evidence based and person centred care to individuals with Mental Health disorders across the localities.

You will be an experienced nurse with the personal drive, clinical knowledge and commitment to undertake this challenging and rewarding role. You will possess highly developed communication skills and have experience of working effectively across professional boundaries. You will be expected to work autonomously and demonstrate proven leadership and change management skills.

The post holder will be a Registered Mental Health Nurse with significant post registration experience and appropriate further education in a relative field including being a qualified Advanced Nurse Practitioner.
