You will be required to undertake responsibility for provision of highly specialist clinical care to community respiratory patients, incorporating patient assessment, diagnosis treatment and discharge from care and lead a team of less experienced clinicians. You will prevent unnecessary or unscheduled acute hospital admissions and provide specialist respiratory care to patients with COPD. You will provide clinical expertise and support to healthcare colleagues working in respiratory care field. You will support the early discharge of COPD patients from hospital. You will provide expert anticipatory care services to patients with COPD at most risk of admission, linking with the Respiratory Early Supported Discharge (ESD) team, and hospital based respiratory assessment services/respiratory nurse specialists. You will be required to assist and develop the service. You should meet the following essential criteria. A Degree or diploma leading to Physiotherapy HCPC registration, current HCPC registration, substantial post graduate experience with relevent working at a senior level within respiratory. You will need evidence of application of acquired skills related to speciality with knowledge of current best practice in chosen speciality. The shift pattern for this post is 5 days over 7.
