An exciting opportunity has arisen to help shape the transformation of Community Physiotherapy Services for COPD patients in Midlothian. The expansion of the Community Respiratory Team has led to the creation of a new B6 Specialist Physiotherapist position. This post will give the successful applicant the opportunity to shape and develop the care for COPD patients in Midlothian. The post holder will be expected to perform respiratory assessments in the patients’ place of residence, develop treatment plans, encourage self management of patients with COPD. They will be expected to carry an independent caseload, work effectively as a team member and be involved in data collection and audit. The focus of the team is on prevention of admission to hospital for COPD exacerbations and improving self management in COPD patients. It aims to facilitate care between hospital and community working by attending regular multidisciplinary meetings in secondary care and communicating regularly with GP colleagues. The post will allow working in structured and supported environment, and the chance to explore new ways of working.
