Working within the community nursing team and under the direction of the District Nurse you will assess and deliver an agreed plan of care to individuals within a community setting. You will be responsible and accountable for assessing, planning and implementation of programmes of care as directed by district nurse. These will be developed in partnership with Patients, Family, Carers and Significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health. It is essential that you are currently a Registered Nurse with the NMC and that you have community nursing experience. This is an opportunity to further develop and enhance clinical and managerial skills, providing support for the District Nurse, with the community environment. The hours of work will be between 5 days over 7 including back shift until 10.30pm and 1 in 4 weekends. Having a current driving licence would be desirable.
