Within a multi-disciplinary team you will have responsibility for assessing, planning, implementing and evaluating the care needs of older adults with Mental Health problems. Emphasis will be on care and treatment using validated psychological approaches and working as an integrated service to meet complex needs of this client group,.  You will be responsible for providing supervision to untrained staff and nursing students. Experience in Older adults Mental Health is desirable.  You will provide a service to a defined geographical area for patients who are experiencing mental health problems and also has a key role in promoting and developing the role of the Community Psychiatric Nurse.  You will provide high quality, culturally sensitive and effective specialist nursing to older people with mental illness living in the community by working in partnership with service users, care providers and other agencies.  You will provide triage, assessment, ongoing interventions and evaluation of these, participates in discharge planning processes and facilitates the coordination of care working collaboratively with patients using a biopsychosocial model within community OPMH services.  You will identify patients with complex needs requiring referral to psychiatric inpatient services, other agencies/teams both primary & secondary and voluntary agencies.  You will provide expert clinical advice, practical support and specialist education to other professional groups/staff regarding the mental health needs of the individuals as well as teaching students. It is essential that you are currently a Registered Mental Health Nurse with the NMC and that you have evidence of post basic professional development.  It would be desirable if you have a current driving licence.  The shift pattern for this post is Monday - Friday 9.00am -5.00pm.
