We are looking to recruit at total of 18 Full Time Community Link Workers (CLWs) within NHS Lanarkshire (across North and South Health and Social Care Partnerships). 

This is an exciting opportunity to work as part of a Primary Care Multi-disciplinary team.  The CLWs will provide a person centred approach in response to the needs and interests of GP practice patients supporting them to identify issues that affect their ability to live well.  The CLWs will work collaboratively with patients supporting them through decision making processes to identify issues that they would like to address and help them to overcome barriers to improving these.


The CLWs will be managed by the respective CLW Co-ordinator and will provide a person-centred service to GP practice patients to help identify and to support them to access community resources. The CLWs will facilitate access to these by establishing and maintaining effective relationships between community resources and GP practices.


The CLWs will  work in partnership with a number of statutory organisations and voluntary services, as well as internal resources.  In addition, they will utilise social inclusion and community mapping approaches to develop links with other agencies within the community and create opportunities for the patients they are supporting.
