The Integrated Community Support Team will provide a coordinated approach to delivering accessible, appropriate, high quality and effective health and social care to people within the community, working with other partner agencies towards the Scottish Government’s 2020 vision, Shifting the Balance of Care and the National Delivery Plan for Allied Health Professionals.


           

Undertake operational management lead for the multi-disciplinary team, in collaboration with other partner agencies, i.e. primary care, social care, acute care, third sector and independent providers.

           

Be accountable for the 24 hour delivery of this service.


Contribute to the ongoing service design, development, implementation and evaluation of the Service.


Registered Nurse with valid NMC Registration or Registered Allied Health Professional with valid HCPC registration


If nurse : District Nurse Certificate/SPQ Nursing in the Home or equivalent


Educated to/working towards/operating at Degree Level


Post graduate expertise within speciality / area of practice that demonstrates the required breadth of knowledge required to lead safety, effectively and efficiently.

 

Comprehensive knowledge of clinical guidelines and standards within the speciality / area of practice.
