The Pharmacotherapy Service in Forth Valley is rapidly expanding and the team are developing new and exciting roles within local GP practices and working towards fulfilling the vision set out by Achieving Excellence in Pharmaceutical Care. This post will therefore be based in GP practices to increase capacity to deliver pharmaceutical care. You will be an integral and valued member of the GP Team delivering the Pharmacotherapy Service.

The post holder will have highly effective interpersonal, communication and influencing skills. They will also have the ability to work with a range of clinical and non clinical personnel as they become core members of the practice teams. They will liaise closely with hospital and community pharmacy colleagues and care homes to ensure seamless pharmaceutical care. In addition, the pharmacist will have or will have the opportunity to develop the necessary clinical skills to enable them to undertake medication reviews. There will also be an opportunity to undertake training to become an Independent Prescriber if required.
