Working within the North West Rehabilitation Service and including the intermediate care unit, this post offers an excellent opportunity to work within a multidisciplinary team providing assessment, treatment, intervention and advice to older and disabled adults with multiple pathology aiming for hospital admission avoidance and supporting hospital discharge. You will work autonomously in a diversity of health and social care settings taking on, in addition to the professional clinical role, case co-ordination, health promotion, audit projects, and the provision of expert advice on professional interventions .
You will manage a complex clinical caseload within the service, applying specialist knowledge and skills relating to their professional role and working as an effective and efficient partner in the interdisciplinary team.
This is a community post and you will work in a variety of locations in the North West.
You will be a HCPC registered Podiatrist. A full driving licence is essential. Experience of being part of a multi-disciplinary team and working in the community is desirable.
The shift pattern is 18.75 hrs over weekdays and occasional weekends. Pattern can be negotiated.
