We have an exciting career opportunity for an experienced, high calibre pharmacist ready to try something new and join the recently established Hospital at Home team. Developing leadership and clinical skills, you will work alongside the current Hospital at Home team in Falkirk Community Hospital with support from the pharmacy department in Forth Valley Royal Hospital, to establish new pharmacy services catering to this unique patient group.  

The hospital at home team is currently made up of nursing staff including ANPs and medical staff who visit patients in their own home on a daily basis, aiming to prevent hospital admission. The service operates seven days a week. Patients may be stepped down to Hospital at Home from FVRH and vice versa.

We are looking for a highly motivated and enthusiastic senior pharmacist to make a difference and contribute to successfully establishing pharmacy services to the patient group being cared for by the Hospital at Home team. This will involve reviewing these patients clinically, facilitating transfer of care across boundaries, supporting medicines supply and ensuring correct and optimal use of HePMA. You will work to ensure medicinal therapy is patient-centred, safe and cost-effective, and aim to prevent hospital admissions due to medicine-related harm. New processes will need to be set up, and you will have a key role in educating both patients and other members of the Hospital at Home team.

You will need to be flexible, organised, work well within a team and have a positive, ‘can do’ attitude.  You need to enjoy problem solving, innovation, working in a challenging environment, be able to manage multiple competing priorities and deliver against deadlines. You will have excellent communication skills, be an enthusiastic educator and have proven multidisciplinary influencing skills. You will be a highly specialist practitioner, with advanced knowledge and the experience to provide high quality patient-centred care. As an expert pharmacist, you will contribute to advanced level decision making on a daily basis.

The pharmacy service within NHS Forth Valley is highly regarded across the organisation with staff working closely within multi-disciplinary teams. Both the Hospital at Home team and wider pharmacy team are forward thinking, friendly, supportive and are constantly seeking improvement.

Ability and willingness to travel throughout the area of service delivery is essential for this post.
