In response to Scotland’s nine national health and wellbeing outcomes, Inverclyde HSCP aims to improve outcomes for people by assisting them to live in their own home for as long as possible and prevent admissions to hospital. In addition, Inverclyde HSCP is supporting the new General Medical Services (GMS) Scotland Contract 2018-2021 through the addition of ANPs working across GP Clusters to support unscheduled care, particularly home visits. There are three GP clusters in Inverclyde co-terminus with our planning localities, these are: East Inverclyde (23,608), West Inverclyde (29,237) & (28,509).
You will assist in coordinating the provision of seamless care, in conjunction with medical staff and to liaise with other members of the Multi-disciplinary Team to facilitate investigation, treatment, transfer or discharge of the patient as efficiently and effectively as possible.
You will also provide a high quality, safe and supportive environment in order to care for patients within NHS Greater Glasgow & Clyde meeting the identified physical and psycho-social needs
You should be a Registered Nurse on the NMC Register and be educated at Masters Level in advanced practice and are assessed as competent in this level of practice. As a clinical leader you will have the freedom and authority to act and accept the responsibility and accountability for those actions. This level of practice is characterised by high level autonomous decision making, including assessment, diagnosis, treatment including prescribing, of patients with complex multi-dimensional problems.
You should also be recorded as a Nurse Independent/Supplementary Prescriber (V300) with the NMC
An appropriate Masters level qualification in Advanced Practice (or working towards*) which has enabled competence in:
Clinical assessment
Clinical reasoning, judgement and diagnostic decision making
Anatomy and pathophysiology
Non medical prescribing- V300
Leading, delivering and evaluating care
Practice learning/transferable work based learning
