We are seeking a highly effective and experienced Primary Care Practitioner who will work closely alongside our GPs holding clinics, home visiting and enhancing our current patient services. You will be a qualified Advanced Nurse Practitioner. You will support the continued development, delivery and management of the Primary Improvement Plan across Borders.
You will contribute to the strategic development for the future model of community health care services. The post holder will work closely with general practitioners, and other members of the primary and community services team to shape and support the implementation of the Primary Care Improvement Plan.
This is an exciting time to join NHS Borders as we embark on a transformation programme for community health care services and the post holder will have a key role in helping to influence the future delivery of these services.
We are looking for a senior Advanced Nurse Practitioner to lead and manage our work on the professional nursing and nursing workforce aspects of the Primary Care Improvement Plan.
